What To Expect

  • Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling. Educate yourself on your symptoms by reviewing the content on this Web site. Take note of the types of shoes that you wear as well as any recent changes in shoe gear which may have brought on you discomfort. Also, if the purpose of your visit is for computer generated orthotics, please be sure to bring a pair of shoes that you will be wearing them in.

  • Plan to arrive 15 minutes prior to your scheduled appointment time. You will need to bring your insurance card, your co-payment, and your primary care physician’s referral if you belong to an HMO or PPO plan that requires a referral for a specialist.

  • You will be asked to complete a patient information form, and a form for your medical history (see links above). In order to have the most complete and accurate information, bring a written list of any medications, including any over-the-counter medications and nutritional supplements you are currently taking. Also be prepared to list prior treatments and surgeries, even if they are not related to your current problem.

  • If you need special assistance, please consider asking a friend or family member to accompany you. Let us know when you schedule your appointment if you have any special needs that we can assist with. We look forward to your first visit!

  • If, for any reason, you cannot keep a scheduled appointment or will be delayed, please call as soon as possible. Charges may be incurred for appointments canceled less than 24 hours before scheduled appointment time.

Insurance

As a service to you, our office staff will file your insurance claims when you have provided us with all the necessary information. We participate in the Medicare program and are members of most of the leading PPOs and HMOs in the Greater Hartford, CT area. We are providers for many other commercial health plans as well. Please contact our office to determine if we are providers for your health plan. Please understand that we cannot guarantee payment of your claim. Patients are responsible for the payment of any deductible and any co-insurance amounts at the time of service. Our office staff and billing company are always available and will be glad to answer any questions.
